Keystone 65 – Health Maintenance Organization (HMO)

Our Keystone 65 plans combine the benefits of Medicare and a typical Medicare supplement with the features of a HMO plan.

Keystone 65 Value, Advantage, Preferred
Keystone 65 Value, Advantage and Preferred are Medicare Advantage HMO plans designed for members who are looking for everything from lower monthly premiums to higher copays or higher monthly premiums to lower copays and everything in between.

Keystone 65 – Point of Service (POS)

Keystone 65 Direct
Keystone 65 Direct is our Medicare Advantage POS plan. Keystone 65 Direct offers the same coverage as our Keystone HMO plans, but gives your employees the freedom to seek care from any provider in or out of the network — in most cases, without a referral.

Personal Choice 65^SM – Preferred Provider Organization (PPO)

Personal Choice 65
Personal Choice 65, our Medicare Advantage PPO plan, gives your employees the freedom to go to any doctor or hospital in the U.S. without a referral. And, unlike our HMO and POS plans, your employees won’t need to select a primary care physician for their care.

Security 65^® – Medigap

Security 65
Security 65 is our series of Medicare Supplement Medical plans (Medigap). Our Security 65 plans combine the benefits of Original Medicare and features of a traditional, fee-for-service health plan.

You can also purchase Select Option, our Medicare prescription drug coverage, with any Security 65 plan.

Private Fee-For-Service

Select Advantage
Select Advantage, our Medicare Advantage PFFS plan, gives you the freedom to choose any physician or go to any hospital. There are no networks and no limitations as long as the health care provider accepts the plan’s payment terms and conditions.

Select Option
Select Option is our Prescription Drug Plan that complements Medicare Supplement Medical plans like Security 65.
